 I'm agreeing with the majority and saying Sledgehammer. Like someone mentioned when the platform is removed from the accused's feet and they drop down the impact breaks their neck, to make sure it does what they used to do is actually get a man to run across the courtyard/field and jump at them and grab onto their feet to pull their body down, to make sure it breaks. While it doesn't mention that in the account of his death I can imagine someone doing that and I dunno... I always imagined that after they tried it you could hear Sledgehammer making this really messed up choking laugh, despite all the blood oozing through the bag... Gross!

Also the way he pulls up on the rope and there's supposed to be a really bad smell, (The account mentions several spectators fainting.) I dunno... It's just really creepy to me.
